Plan A Visit Summer Mini-Camp

Photo Permission and General Release of Liability: I hereby authorize NAHF and its affiliates to publish any photographs or videos taken of me or my minor child(ren) for use in NAHF’s printed publications, website, and other mediums, and for training purposes. I release NAHF and its affiliates from any expectation of confidentiality and attest by my signature that I am the parent or legal guardian of the child(ren) listed and that I have authority to permit NAHF to use said photographs and/or video for promotional and training purposes. I acknowledge that participation in the event is voluntary. I acknowledge that NAHF confers no rights in photos and/or video of my minor child(ren) and that neither me nor my minor child(ren) will receive financial or other compensation for use of such photo and/or video. I hereby release NAHF and its affiliates from liability for any and all claims that may arise in connection with participation in Discovering Flight events, including but not limited to liability that may arise from any related publication or broadcast.
